Return of Premium (applicable to St. Peter Life Plan Plus only) subject to the conditions Life Plan Contact.

It's easy! You just have to present the following documents to any St. Peter Life Plan, Inc. office and they will process everything else for you! Each document is strictly released only to your family. WE PROTECT AND SAFEGUARD YOUR BEST INTERESTS.
The following are the requirements for each specific benefit from your St. Peter Life Plan:
Requirements in availing of the Memorial Service:
Life Plan Agreement (LPA) - Planholder's copy & Policy Card
Certificate of Fully Paid Plan (if fully paid)
Branch Statement of Account
If assigned, 1 valid ID of Planholder plus an Assignment Fee.
Dully filled out Memorial Service Call Slip
If assigned, 1 valid ID of Planholder plus an Assignment Fee.
Death Certificate

Requirements to claim your Cash Assistance Benefit and Extended Cash Assistance Benefit:
Death Certificate with seal and issued by the Local Civil Registry or National Statistics Office (NSO)
Statement of Claimant/Beneficiary
NSO Authentication Birth Certificate of Planholder and Claimant/Beneficiary
Marriage Contract of the Beneficiary (if the claimant is the spouse)
Photocopy of Valid ID's of the Beneficiary
Branch Statement of Account
Physician's statement (if within contestability eriod and cause of death is due to illness)
Life Plan Agreement (LPA) - Planholder's copy
Certificate of Full Payment of Plan (if fully paid)
Policy Card
Medical History (if death happened within the contestability period)

Dully accomplished Cash Surrender Value/Termination Value Application Form
At least two (2) valid ID's showing the signature of the Planholder (SSS, Driver's License, Company ID, etc.)

Marriage Contract of Planholder (if there is a change in the last name of the Planholder as indicated in the LPA)
